States we cannot serve — please read before shipping

Prime Path Lab cannot accept specimens from patients located in:

  • New York
  • California
  • New Mexico
  • Alabama
  • Nevada

If your patient resides in one of these states, we cannot perform testing. Please do not ship — contact us and we will help identify an alternative.

Tell us what you need and we will ship it. Kits, containers, requisition forms, and prepaid return packaging.

This form must never collect patient information

No names, no dates of birth, no diagnoses, no insurance identifiers. A supply request is a practice-level transaction. The moment it collects PHI, this website becomes a system that handles protected health information, and the hosting arrangement behind it does not support that.

Supply request form pending — build in Fluent Forms with: practice name, ordering physician, NPI, shipping address, kit type, quantity, contact name, contact email, contact phone.

How to order

Five steps.

  • Complete the Prime Path requisition form (PDF pending)
  • Collect the specimen per handling requirements for that test
  • Label the specimen with two patient identifiers
  • Ship per the instructions on the requisition
  • Results delivered via portal, fax, or HL7 interface
